Self Pay Rates:
My fee for a standard individual therapy session is $105.
Reduced fee rate is available and will be made in a collaborative discussion about an individual’s or couple’s income and expenses.
I am currently in-network with
– TRICARE West
– Kansas Medicaid
For all other insurance plans, I am an Out-of-Network provider. I am happy to provide you with a superbill that you can submit to your insurance company for possible reimbursement. It is your responsibility to know your benefits and the section below can help guide you in what questions to ask your insurance company to know what your plan covers and if you can be reimbursed for out-of-network benefits.
I accept cash, checks, HSA cards and credit cards for payment. Payment is due at time of service and I will request a credit card be kept on file. Unless otherwise requested, your session fee will be charged to the the card on file. This includes self-pay, co-pay, co-insurance and deductibles.
You are responsible for attending each appointment and agree to adhere to the following policy: If you cannot keep the scheduled appointment, you MUST notify my office to cancel or reschedule the appointment within 24 hours of the scheduled appointment time. Failure to adhere to this 24-hour cancellation policy will result in a charge of 50% of the full session fee: $45. Missed appointment charges will automatically be charged to your card unless other arrangements are made.
Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.
I’d recommend asking these questions to your insurance provider to help determine your benefits:
- Does my health insurance plan include mental health benefits?
- Do I have a deductible? If so, what is it and have I met it yet?
- The therapist I have chosen is out-of-network and I can provide a superbill for reimbursement, how much will you reimburse me for X amount of the bill?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- Do I need written approval from my primary care physician in order for services to be covered?
